Fees

Most insurances are accepted, including employee assistance programs. You may have a co-payment. Please contact your insurance company or employee assistance program to ensure Dr. Stewart and Dr. Congdon are members of their panel of providers, as well as to determine your copay and deductible.

Sliding scale fees are available, should you not have insurance coverage. Those fees are based on household income (which includes the combined income of all family members living in your household). Bring documentation of household income to your first appointment in order to qualify for a reduced rate.

When an appointment is scheduled, that time is being committed to you. Unless your appointment is cancelled with at least 24 hours notice, you may be charged $65.00. This charge is not billable to your insurance. It also falls outside any sliding scale fee structure you may qualify for.
